Note: If the use case selected is conversational in nature, omit any mention of promotional messages and ensure only applicable use cases are listed

Example Message:

“Hi, this is a reminder for your upcoming appointment with Dr. Palumbo at Precision Telemed on 4/11/2025 at 5:00PM EST. To stop receiving these messages, reply STOP at any time.”

3. Message Frequency:

The number of messages you receive may vary depending on the nature of communication. For instance, you may receive up to 4 SMS messages per week related to your appointment reminders or promotions.

Example:

“Message frequency may vary. You may receive up to 4 SMS messages per week regarding appointments or account activity.”

4. Messaging Fees:

Standard message and data rates may apply, depending on your wireless carrier’s pricing plan. Charges may differ based on whether messages are sent domestically or internationally.

5. SMS Opt-In Procedures:

You may provide consent to receive SMS messages from Precision Telemed through one of the following methods by completing and submitting an online form.

6. How to Opt-Out:

You may unsubscribe from SMS communications at any time by replying “STOP” to any text message. Alternatively, you can contact Precision Telemed directly to request removal from the SMS distribution list.

If you do not wish to receive SMS messages, simply refrain from selecting the SMS consent checkbox on our forms.

8. Protected Health Information and SMS:

By agreeing to our Terms and Conditions, you are authorizing Precision Telemed to send you SMS messages that may contain Private Health Information. This information will include appointment reminders, medication refill reminders, and general account notifications. At no point in time will Precision Telemed send clinical health history via SMS.
